Comparative study on conventional D-J stenting versus non-stenting following uncomplicated lower ureteric pneumatic lithotripsy

Abstract:

Urolithiasis is 3rd most common illness of urinary tract with a prevalence rate of 7.8%. Improvements in ureteroscope design, including flexibility, downsizing and advancement in intracorporeal lithotripsy have made the procedure easier with high success rates and minimum morbidity. The aim of this study is to compare the outcome between conventional DJ stenting versus non-stenting after uncomplicated lower ureteric pneumatic lithotripsy. This quasi-experimental study was conducted at the Department of Urology, Dhaka Medical College Hospital (DMCH) for a period from January’2021 to june,2022. A total of 60 patients with lower ureteric stone were selected who underwent uncomplicated ureteroscopic pneumatic lithotripsy and were divided into two groups through non-randomized technique by odd and even number. Mean age ± SD of stented group was 35.17 (±10.82) years and mean ± SD age of non-stented group was 37.8 (±9.19) years. The age differences were not statistically significant (p value = 0.169). In stented group and non-stented group male were 17 (56.7%) and 18(60.0%) respectively. In stented group, frequency was found 53.3% where as in non-stented that was 16.7%. Urgency was 43.3% in stented and 13.3% in non-stented group. Flank pain was present 46.7% in stented group where 13.3% in non-stented patients. In stented patients’ hematuria was found 10.0% in stent group where as 3.3% in non-stented group. Urinary tract infection (UTI) was found 16.7%,6.7% and 6.7% in stented group following 1st,4th and 12th weeks after surgery where as 3.3%,3.3% and 3.3% in non-stented group respectively. This study reveals rroutine placement of ureteral stent is not beneficial in uncomplicated ureteroscopic pneumatic lithotripsy for lower ureteric calculi.
